If in our judgment, your dog requires medical care, you agree to be solely

responsible for the payment of all medical bills for your dog and you release

Canine Community College, its officers, directors, agents, and employees

(“Canine Community College”) of and from any and all responsibility for, or

claims, damages, debts, arising out of or related to such medical care, including

but not limited to, transportation to/from the veterinarian clinic and choice of

veterinarian or animal hospital.

Additional Canine Community College Policies:

 

1.  Canine Community College reserves the right to immediately change your

dog’s type of boarding/daycare if we believe it is necessary to protect the

health and well being of your dog, other dogs, or a staff member. 

 

2.  Canine Community College cannot guarantee that toys, blankets, or bed will

be kept in the same condition as brought in. For safety reasons, tennis balls

and rawhide are NOT allowed.

 

3.  All dogs, over 8 months old, must be spayed or neutered, and must have all

their vaccinations. 

 

4.  All dogs must have an identification tag with either a phone number or

address of the owner. We recommend that the tag include the dog’s name.

All dogs must be on a leash when entering or leaving the Bark Park

 

5.  There is a $2 per day charge when feeding your dog with “house” food

(House food is Purina Pro Plan Chicken and Rice) NOTE: We recommend that

you bring in your own food since changing a dog’s food can cause a severe

upset stomach. 

 

6.  Failure to cancel your reservation at least 48 hours before check in

date, or failure to show up for reservation, will result in charge of ONE NIGHT

of BOARDING.

 

7.  Dog owner understands the risks involved with communal boarding of dogs.

Our boarding, Daycare, and grooming dogs as well as Obedience Training

takes place in the yard and inside with other dogs. Although we will offer

reasonable care, the unpredictable personality of dogs can sometimes lead to

injury. 

 

8.  Dog owner understands that Canine Community College’s liability, of any

circumstance related to the dog, will not exceed the current chattel value of a

dog of the same breed and quality as the one in our care.


 

2010 Canine Community College Boarding & Daycare Agreement

 

Our policy regarding BOARDING charges is as follows; A boarding charge applies

when a dog stays in our facility OVERNIGHT.

 

Upon a dog’s departure, a Daycare charge is added only when the dog’s

checkout time is more than four hours AFTER its check-in time. 

 

EXAMPLE: A dog that checks in on a Monday at 8:00 am and checks out Tuesday

at 5:00 pm, would be charged for ONE night of boarding and TWO days of

Daycare. If the same dog checked out at 10:00 am, then they would only be

charged for the ONE day of daycare and ONE night of boarding. Please refer any

Payment must be made when services are rendered.

 

Canine Community College reserves the right, without notice, to adjust its fees

for services. Please inquire with Erika Morgenstern as to our current fees.

 

By submitting this form;

 

You indicate your agreement with all the terms hereof,

 

You acknowledge the risks of communal boarding. Dogs Obedience Training

together in Obedience Training groups can sometimes result in injuries or

spreading of such illnesses as Kennel Cough.

 

You authorize Canine Community College to obtain medical and vaccination

records for your dog from the veterinarian listed above and you hereby

authorize your veterinarian to provide these records to Canine Community

College.

 

You release, indemnity and hold Canine Community College blameless from

any and all manner of damages, claims, losses, liabilities, costs or expenses,

causes of action or suits, whatsoever in law or equity (including, without

limitation, attorney’s fees and related costs) arising out of or related to the

services provided by Canine Community College, except which may arise from

the sole gross negligence or intentional and willful misconduct of Canine

Community College, including, without limitation: (i) any inaccuracy in any

statement made by yourself or information provided by you to Canine

Community College (ii) your dog, including but not limited to destruction of

property, dog bites and transmission of disease, and (iii) any action by

yourself which is in breach of the terms and conditions of this Agreement.


 

This agreement covers the current relationship between the Canine

Community College and yourself. Each time you bring your dog to the Canine

Community College, you affirm the terms of the agreement and the

truthfulness and accuracy of all statements you make in this Agreement.

 

Please remember to call the location directly for boarding and/or Daycare

reservations.

 

To ensure your dog’s safety, as well as that of our existing Canine Community

College dogs, you are required to bring a copy of your dog’s updated shot

records from your vet before you are allowed to start with us.

This is a contract between Canine Community College and the pet owner whose

signature appears below (hereinafter called “Owner”).

 

· Owner agrees to pay for Daycare of their pet at the rate in effect on the date

of attendance at either: a daily or frequent visit pass. 

· There are no refunds for Daycare passes unless Canine Community College 

determines that dog client is unfit for our facility due to improper behavior. 

· Owner further agrees to pay all costs and charges for special services

requested including, but not limited to late pickup charges, and all veterinary

costs for the pet during the time said pet is in the care of Canine Community

College. 

· For boarding clients, pickup times are between 7:30am J 9am Monday thru

Friday and 9am J 11am Saturday and Sunday, in order to avoid additional

Daycare charges. You will be charged a day of Daycare if you pickup your

dog after those hours, but then your dog can enjoy a full day of Daycare up

to closing hours posted.. 

· n addition, owner agrees that the owner or agent for the owner will pick up

their pet(s) prior to 6:30 pm Tuesday– Friday, 4:00pm Saturday and Sunday

unless prior arrangements are made or a late fee will be due billed at $5.00

per hour or partial hour. If a pet(s) is not picked up by 4:00 pm weekends or

6:30 pm weekdays their pet(s) may be boarded overnight at owners expense

at the current rate in effect. Owner agrees to pay any additional charges

upon pick up of their pet(s).

· Owner agrees to pay for any and all property damages that pet may destroy

while in the kennel. Owner also understands that pet may be ‘crated’ in the

event of property destruction for the prevention of further damages.

 

· By signing this contract and leaving their pet with the Canine Community

College , Owner certifies to the accuracy of all information given about said

pet(s) both in this document and in the Client Information Form, and Medical

Release Form. Owner also agrees to the terms stated in the Day Care

Guidelines, Client Information Form, Day Care Agreement, and Medical

Release Form all incorporated herein by reference.

· Canine Community College shall exercise due diligence and reasonable care

for the pet(s) delivered by the owner to Canine Community College for

Daycare services. All pets are handled or cared for by the Canine Community

College Staff without liability on the Facility’s part for loss or damage from

disease, theft, fire, death, escape, injury or harm from any other unavoidable

causes, due diligence and care having been exercised. It is expressly agreed

by Owner and Canine Community College that Canine Community College

liability shall in no event exceed the lesser of the current value of a pet of the

same breed, or the sum of $250.00 per animal left by owner for Daycare

whichever is less. 

· The Owner of the pet further agrees to be solely responsible and liable for

any and all acts or behavior of said pet while pet is in the care of Canine

Community College.4. Owner specifically represents that he or she is the sole

owner of the pet, free and clear of all liens or encumbrances.

· Client agrees to pay any reasonable attorney fees incurred by Canine

Community College to collect any charges incurred by the owner.

· Owner specifically represents to Canine Community College  that the pet has

not been exposed to rabies, bordetella, or other communicable diseases

within any thirty day period prior to boarding or Daycare, and that all

vaccinations are current and will be maintained as outlined in the Client

Information Form. Pets arriving with fleas or ticks will be bathed at the

owner’s expense. Canine Community College reserves the right to refuse any

dog. 

· All charges incurred by Owner shall be payable prior to drop off of the pet, or

when billed by Canine Community College  at address listed on contract. In

the event of a check returned for insufficient funds or the denial of a credit

charge, the client agrees to pay all charges incurred by Canine Community

College to collect this debt plus an additional $25.00. Client may then be

placed on a cash only account basis based on management’s decision.

· If pet, becomes ill or if the state of the animals health otherwise requires

professional attention, Canine Community College , in its sole discretion, may

engage the services of a veterinarian, administer medicine or give other

requisite attention to the animal, and the expense thereof shall be paid by

the Owner.

· Owner agrees that their pet(s) maybe videotaped, photographed and

recorded while they are attending Canine Community College . Canine

Community College shall be the exclusive owner to the results and all

proceeds of such tapings, photography, and recordings with the rights,

throughout the world, an unlimited number of times in perpetuity, to

copyright, to use and to license to others in any manner. The owner further

agrees that their pet(s) may be used in any and all media and in the

promotion, advertising, sale and publicizing, and exploitation of Canine

Community College  at no cost to Canine Community College.

· This contract contains the entire Agreement between the parties. All terms

and conditions of this contract shall be binding on the heirs, administrators,

personal representatives, agents, and assigns of the Owner and Canine

Community College.  Any part of this contract found to be invalid shall not

affect the remainder of this contract, which shall remain in full force. 

· I have read, understand, accept, and agree to be bound by the terms, and

conditions of this Agreement. Any controversy or claim arising out of or

relating to this contract, or the breach thereof, or as the result of any claim or

controversy involving the alleged negligence by any party to this contract,

shall be settled in accordance with the rules of the American Arbitration

Association, and the judgment upon the reward rendered by an arbitrator

may be entered in any Court having jurisdiction thereof. The arbitrator shall,

as part of his award, determine an award to the prevailing party of the costs

of such arbitration and reasonable attorney’s fees of the prevailing party.

RULES & REGULATIONS

 

The goal of our daycare is to provide a safe, fun and stimulating social

environment for dogs during the day and evening. We believe that this new pet

care option for busy pet owners will open up new homes for more shelter dogs. 

 

To ensure the safety and health of your pet and of our other guests, we require

all of our guests to comply with the following Rules and Regulations.

 

SHOTS: All dogs must have up-to-date vaccinations. Owners must provide

written proof that their dogs have received DHPP (Distemper, Hepatitis,

Parainfluenza, and Parvovirus) within the last year and the Kennel Cough

(Bordetella) vaccine within the past six months. Rabies must also be current as

required by local law.

 

HEALTH: All dogs must be in good health. Owners will need to verify that their

dog(s) are in good health and have not been ill with a communicable condition in

the last 30 days. On admission all dogs must be free from any conditions, which

could potentially jeopardize other guests. All dogs will require a veterinarian

certification of health to be admitted or readmitted after any illness.

 

 

BEHAVIOR: All dogs must be non-aggressive towards humans and other dogs.

They must not be object or food possessive or aggressive. Owners will need to

certify that their dog(s) have not harmed or shown any aggressive or threatening

behavior towards any person or other dog(s). Please remember that your pet will

be spending time with other pets and that their safety and health is our main

concern.

DAYS & HOURS: We are open Tuesday– Friday from 7:30am to 6:30pm. 

Weekends are available upon request.  Holiday hours may differ. Dogs dropped

off prior to 7:30 AM will be charged 5.00 per hour or partial hour early.  Dogs in

the facility after 6:30 pm will incur a 5.00 per hour or partial hour late fee.

 

BILLING POLICIES: Payment is expected prior to your dog being dropped off.

Holidays will require 50% payment up front to secure the reservation at

time of booking. Cancellation of holiday reservation less than six weeks prior to

check-in will be charged a rate of 50%. Non-holiday cancellation policy will

default to Page 1, Section 1. We are a small business and appreciate your

prompt payment.

Medical Release Form

 

Dear Client:

 

In the event that our staff deems your dog is in need of immediate veterinary

care, or your dog presents with a potential illness and we are unable to contact

you, or you are unable to pick up your dog we will take your dog to a licensed

veterinarian. If your personal veterinarian is located in Oakland we will make

every attempt to have your dog taken to them. If your veterinarian is

unavailable, or we deem that your dog needs immediate care, or your

veterinarian is out of our area, we will transport to the nearest available

veterinary clinic.

Your dogs activity level will be heightened during their stay with us. Depending

on the breed of our guest, you might want to consider an increase in their

normal feeding plan. Keep in mind, they will be in the play group for up to 12

hours a day (with naptime), socializing and Obedience Training. Sometimes that

results in a very active, hungry guest at the end of their fun day. We want

everyone to be comfortable in every way while they are vacationing here at

Canine Community College.

Water is given at night unless otherwise requested

Medication, Special meals & House food available at an additional charge

Tranquil music is played at night, ambient lighting turned on and

nighttime treat given to ensure a relaxing transition to sleep
